Pharmacological action
Nexium (Esomeprazole) reduces acid secretion in a stomach by specific inhibition of the acid pump in parietal cells. Nexium (Esomeprazole) is the weak basis, concentrates and passes in the active form in strongly acidic medium of secretory canaliculuses of parietal cells of a mucosa of a stomach. Nexium (Esomeprazole) is unstable in acidic medium, therefore for peroral application use the tablets covered with a cover steady against action of a gastric juice and containing granules of a preparation. The preparation is quickly absorbed: the maximum concentration in plasma are reached in 1-2 hours after reception. Absolute bioavailability of a preparation at repeated reception of Nexium (Esomeprazole) 20 mg unitary a day makes 89 %. Contacts fibers of plasma on 97 %. Nexium (Esomeprazole) is exposed to a full metabolism with participation of system of cytochrome P-450 (CYP). The semideducing period makes 1,3 hours at regular reception in a dosage regimen unitary a day. It is completely deduced from a blood plasma and does not show the tendency to accumulation. At peroral application to 80 % of a dose it is deduced in the form of metabolites with urine, the residual quantity is deduced with excrements.
Indications to application
Treatment of an erosive reflux-esophagitis; - long supporting treatment of patients with the cured esophagitis for prevention of relapses. Treatment of an ulcer of a duodenum, Helicobacter pylori; - the prevention of relapse of round ulcers at patients with a peptic ulcer.
Contraindications
Hypersensibility to Nexium (Esomeprazole), replaced to ingredients of the medicinal form to preparations.
Side effect
The preparation is transferred well. In rare instances can be observed - a headache, an abdominal pain, a diarrhoeia, a meteorism, a nausea, vomiting, a constipation, allergic reactions.
Interaction
The lowered acidity in a stomach at treatment by Nexium (Esomeprazole) can lead to depression or rising of absorption of other preparations in the event that the absorption mechanism depends on acidity of medium. Nexium (Esomeprazole) inhibits CYP2C19 - the basic enzyme. Accordingly, joint Nexium (Esomeprazole) application with other preparations in which metabolism takes part CYP2C19, such as Diazepamum, Imipraminum, Phenytoinum, etc., can lead to rising of concentration of these preparations in plasma, that, in turn, will lead to necessity of a dose decline.

Special indicatings
The patients accepting a preparation during the long period (especially more than a year), should be under regular observation of the doctor. At preparation appointment pregnant women should show care. It is not known, whether it is allocated Nexium (Esomeprazole) with thoracal milk, therefore it is not necessary to prescribe Nexium (Esomeprazole) in feeding by a breast.
