Structure
Active substance - Allopyrinolum.
Pharmacological action
Inhibits a xanthine oxidase, breaks transformation of a hypoxanthine into a xanthine and a xanthine - in urinary acid. Lowers the maintenance of lithates in blood serum and prevents their adjournment in organism tissues. Almost it is completely absorbed from GASTROINTESTINAL TRACT. The maximum concentration is reached in 1,5 hours. The semideducing period - 1-2 hours. It is deduced by kidneys and through an intestine.
Indications to application
Hyperuricemia: a primary and secondary gout, an urolithiasis with formation of urate concrements, the diseases accompanied by strengthened disintegration of nucleoproteids, cytostatic and radial therapy of tumours, a psoriasis, a traumatic toxicosis, therapy by corticosteroids for the prevention of an urate nephropathy; malignant neoplasms and congenital disturbances of a purine exchange at children.
Contraindications
The hypersensibility, the expressed renal insufficiency, pregnancy and lactemia.
Side effect
A thrombocytopenia, an agranulocytosis, an aplastic anaemia, a febricula, an asthenia, a headache, a nausea, vomiting, taste disturbance, a stomatitis, a fever, drowsiness, a coma, depression, a paralysis, paresthesia, visual disturbances, a cataract, a stenocardia, a bradycardia, a hypertensia, edemas, an alopecia, a furunculosis, a hyperglycaemia, a lipidemia, a hematuria, an uremia, a gynecomastia, sterility, an impotency, dermal allergic reactions, a Quincke's disease.
Interaction
Raises concentration in bloods and toxicity of Azathioprinum, Mercaptopurinum, Methotrexatum, xanthines (Theophyllinum, Aminophyllinum), hypoglycemic effect of Chlorproramidum, anticoagulative - indirect anticoagulants. Pyrazinamidum, Salicylases, uricosuric agents, Furosemidum.
Overdosage
Symptoms: giddiness, a nausea, vomiting, a diarrhoeia, an oliguria. Treatment: an artificial diuresis, hemo a peritoneal dialysis.
Special indicatings
It is necessary to support a diuresis at level not less than 2 l a day and neutral or alkalescent reaction of urine. It is not recommended to begin therapy before full cupping of an acute attack of a gout; in case of development of an acute attack of a gout against treatment in addition accept resolvents. At disturbance of function of kidneys and a liver it is necessary to lower a dose. Use of Azathioprinum or 6-Mercaptopurinum against Allopyrinolum supposes 4 multiple depression of their doses.
