Aldactone (Spironolactone)Aldactone Pharmacological action
Potassium a diuretic, the antagonist of Aldosteronum. Diuretic an effect has, raises deducing of ions of sodium and reduces deducing of ions of a potassium and magnesium, lowers acidity of urine. The diuretic effect of a preparation is expressed moderately, shown usually for 2-5 day of treatment and proceeds within 2-3 days after the termination of reception of a preparation.
Weak hypotensive action is shown usually on 2-3 week of treatment. The hypotensive effect does not depend on renin level in a blood plasma and is not shown at a normal BP.
Aldactone Indications
- A hydropic syndrome at a chronic heart failure, a cirrhosis, a nephrotic syndrome;
- An arterial hypertensia;
- A chronic heart failure;
- An idiopathic hyperaldosteronism;
- A primary hyperaldosteronism (diagnostics and treatment);
- A malignant hypertensia (as a part of the combined therapy);
- A hypopotassemia;
Aldactone Dosage regimen
The adult at an arterial hypertensia, a hydropic syndrome the preparation is prescribed in a daily dose by of 50-100 mg. Frequency rate of reception of a preparation - 1-2 times/days If necessary Aldactonum dose can be enlarged gradually to 200 mg/days Duration of course reception of a preparation - not less than 2 weeks.
At a chronic heart failure treatment by Aldactonum as monotherapy there begin with a dose of 100 mg/days If necessary a preparation dose the Maintenance dose can be enlarged to 200 mg/days - 25-200 mg/days. If necessary Aldactonum dose can be enlarged to 50 mg/sut or is lowered to 25 mg in day.
At a cirrhosis, in case of a parity To +/Na + in urine more than 1, the preparation is prescribed in a dose by of 100 mg/sut, in case of a parity To +/Na + in urine less than 1 daily dose makes 200-400 mg. A preparation maintenance dose establish individually in each concrete case.
At a nephrotic syndrome the preparation daily dose makes 100-200 mg.
At an idiopathic hyperaldosteronism the preparation is prescribed in a dose by of 100 mg/days
To children Aldactonum is prescribed in a daily dose of mass of a body by of 3 mg/kg. Frequency rate of reception - 1-2 times/days
Aldactonum should be accepted after meal and the optimal is unitary reception of a preparation for the night, proceeding from a rhythm of daily secretion of Aldosteronum.
Aldactone Side effect
From the alimentary system: a nausea, vomiting, a diarrhoeia, spastic abdominal pains, a gastritis, an ulceration, a gastric bleeding.
From party TSNS: a headache, drowsiness, slackness.
From hemopoiesis system: a leukopenia, a thrombocytopenia.
From endocrine system: at long application cases of a gynecomastia, erection disturbance are described; disturbances of a menstrual cycle, uterine bleedings in a postmenopause.
Allergic reactions: a dermal eruption.
Aldactone Contraindications
- Acute renal insufficiency;
- A hyperpotassemia;
- I trimester of pregnancy;
- A hypersensibility to a preparation.
Pregnancy and lactemia
Aldactonum is contraindicative to application in I trimester of pregnancy.
In II and III trimesters of pregnancy a preparation apply only under strict indications, having weighed risk and advantage of its application.
In case of application of a preparation in a lactemia it is necessary to stop thoracal feeding.
Aldactone Special indicatings
For the purpose of the occurrence prevention at application of a preparation of a hyperpotassemia, a hyponatremia and as hyperuricemias at patients with disturbance of function of kidneys becoming perceptible earlier, recommend to spend periodic definition of the maintenance of electrolytes and urea to bloods.
During treatment by Aldactonum it is not necessary to prescribe to patients the preparations containing a potassium.
At Aldactonum application simultaneously with digoxin there can be a necessity of reduction both sating, and a maintenance dose of the last.
Aldactone Overdosage
Symptoms: a nausea, vomiting, giddiness, a diarrhoeia, a dermal eruption, a hyperpotassemia, a hyponatremia, a hypercalcemia, rising of the maintenance of urea in blood of the patient, a dehydration.
Treatment: immediate cancellation of a preparation, a gastric lavage, symptomatic therapy of a dehydration. Hyperpotassemia elimination is reached by fast introduction of a glucose (5-20 % solutions).
Aldactone Medicinal interaction
Aldactonum can strengthen action of diuretics or other hypotensive preparations (at simultaneous appointment the dose of the last should be reduced on the average by 50 %).
Spironolactonum reduces sensitivity of vessels to Noradrenalinum that it is necessary to consider at carrying out of the general and local anaesthesia at the patients receiving Aldactonum.
At simultaneous application of Aldactonum with anticoagulants action of the last is weakened.
Spironolactonum enlarges the period of semideducing of digoxin that can lead to rising of concentration of digoxin in plasma up to the toxic.
Aldactone Conditions and periods of storage
The preparation should be stored in a dry place at temperature not above 30 C. The Period of validity - 5 years.
