Cardizem (Diltiazem)Pharmacological action
Selective blocker of calcium channels of III class. Reduces entering of ions of calcium on slow channels in cells of a myocardium and a smooth musculation. Antianginal, hypotensive and antiarrhythmic an effect has.
The action mechanism at a stenocardia is caused by expansion of coronary arteries and simultaneous reduction of an afterload by heart.
Cardizem reduces systolic and diastolic pressure (not rendering influence on a normal BP) cos expansions of peripheric vessels.
The stable effect develops in 4 weeks after the application beginning.
Cardizem oppresses a calcium current in cells sinus and AV knots, having therapeutic an effect at supraventricular arrhythmias.
Cardizem does not cause electrolytic or metabolic disturbances, does not render negative action on lipide structure of blood.
Indications
- An arterial hypertonia;
- A stable and astable stenocardia;
- A vasospastic stenocardia;
- Therapy after a myocardial infarction;
- Supraventricular disturbances of a rhythm of heart;
- A pulmonary hypertensia.
Dosage regimen
Establish individually.
At treatment of an arterial hypertonia and a stenocardia prescribe 120-360 mg/sut in 3-4 receptions.
At supraventricular arrhythmias prescribe in a dose of 240-360 mg/sut in 2 receptions.
At a pulmonary hypertensia the maximum daily dose makes 720 mg.
Side effect
Probably: a headache, giddiness, an erubescence.
Seldom: a nausea, vomiting, a constipation, transitional rising of activity of hepatic transaminases, edemas of the bottom extremities.
Frequency of occurrence of Cardizem side-effects makes about 5 %.
Contraindications
- AV blockade II and III degrees;
- An arterial hypotension;
Pregnancy and lactemia
In need of appointment of a preparation in pregnancy it is necessary to correlate prospective advantage for mother and potential risk for a foetus.
Special indicatings

Medicinal interaction
Hypotensive Cardizem effect amplifies at simultaneous application with diuretics.
At simultaneous application of digoxin negative inotropic Cardizem effect decreases.
At simultaneous Cardizem application and beta adrenoblockers mutual intensifying of antianginal effect in some cases is noted, however at the given interaction the risk of development of disturbances of conductivity and contractility of a myocardium is raised.
At simultaneous Cardizem application can enlarge concentration of digoxin in Serum.
Conditions and periods of storage
