Hytrin (Terazosin)Pharmacological action
Blocker of a1-adrenoreceptors. Blocking a1-adrenoreceptors of unstriated muscles of a triangle and a neck of a bladder, a proximal part of an urethra and a prostate, Hytrin reduces resistance to a current of urine and normalises an emiction at patients with a good-quality hyperplasia of a prostate. Does not render influence on the size of a prostate.
The preparation causes expansion of arterioles and venules. Reduces venous return to heart, reduces pre-both an afterload on heart and a BP. Long Hytrin treatment usually does not cause development of a reflex tachycardia. The preparation slightly influences warm emission, perfusion of kidneys and rate of a glomerular filtration.
The expressed therapeutic effect develops through 3 h after preparation reception. After reception of a single dose the hypotensive effect remains during 24 h.
Indications
- A good-quality hyperplasia of a prostate;
- An arterial hypertensia (in the form of monotherapy or as a part of the combined therapy).
Dosage regimen
At a good-quality hyperplasia of a prostate the preparation is prescribed in an initial dose of 1 times/days by of 1 mg It is recommended to double a dose with intervals in 1 week before maintenance dose achievement. A maintenance dose - 5-10 mg. The maximum daily dose - 20 mg.
With renal insufficiency and the patient of aged age of correction of a dose it is not required to patients.
At an arterial hypertensia the initial daily dose makes 1 mg (for the night). In order to avoid development of "effect of the first dose" the initial dose should not exceed 1 mg. The daily dose augmentation should be spent gradually. The maintenance dose makes 1-5 mg of 1 times/days
Tablets should be accepted entirely, not chewing, irrespective of food intake.
Side effect
From cardiovascular system: in the treatment beginning development of "effect of the first dose" is possible. The given condition is shown by depression of a BP, an orthostatic hypotension which becomes perceptible at 1 % of patients (mainly at fast rising of a dose, and also at combined Hytrin application with diuretics and-or other antihypertensive preparations).
From party TSNS: giddiness, a headache, drowsiness, sight disturbances.
From the alimentary system: a nausea.
The other: an asthenia, peripheric edemas, augmentation of mass of a body, an impotency, a hemodilution.
Contraindications
- A hypersensibility to Terazosin or to other alpha adrenoblockers.
Pregnancy and lactemia
In need of application of Hytrin preparation at pregnancy and in a lactemia (thoracal feeding) it is necessary to estimate prospective advantage for mother and potential risk for a foetus or the baby.
Special indicatings
With care it is necessary to prescribe Hytrin to the patients having the indicatings in the anamnesis on predilection to development of an orthostatic hypotension.

For prevention of development of an orthostatic hypotension to begin treatment of 1 mg follows from a dose.
The patient should be informed that the risk of development of giddiness, disturbances of coordination of movements, a syncope is enlarged at the long standing, the raised exercise stress, hot weather and the alcohol use.
It is necessary to consider, that at Hytrin application as a part of the combined therapy the initial daily dose should not exceed 1 mg. At additional appointment of diuretics and-or antihypertensive preparations (are applied in low doses) it is recommended to reduce Hytrin dose.
Influence on ability to driving of motor transport and to management of mechanisms
In the beginning of treatment and at augmentation of a dose of a preparation patients are not recommended to be engaged in potentially dangerous kinds of activity demanding special attention and speed of psychomotor reactions (including driving of vehicles).
Overdosage
Symptoms: an arterial hypotension, disturbance of coordination of movements, a syncope.
Treatment: the patient should give horizontal position with the raised feet. Spend symptomatic therapy. The specific antidote is not present. The hemodialysis is noneffective, since Terazosin in high degree contacts fibers of a blood plasma.
