Plendil (Felodipine)Pharmacological action
Possesses sharp selectivity concerning a smooth musculation of arterioles. The action mechanism consists in inhibition electric activity cells, influencing on calcium channels of cellular membranes. Plendil leads to reduction of an afterload by heart. In therapeutic Plendil doses does not influence contractility and conductivity of a myocardium.
As the preparation does not render effect on a smooth musculation of veins or an adrenergic vasculomotor innervation, it does not cause an orthostatic hypotension.
There is a depression of vascular resistance of kidneys. The normal glomerular filtration does not change. The preparation does not influence a daily egestion of a potassium and an egestion of an albumin with urine. At patients with disturbance of filtrational function of kidneys against Plendil reception rising of rate of a glomerular filtration becomes perceptible.
Plendil is effective at all degrees of an arterial hypertensia. BP depression occurs through 2 h after reception of the first dose and lasts during 24 h.
Long antihypertensive Plendil treatment leads to regress of already developed hypertrophy of a left ventricle.
Plendil antianginal an effect has. Reduces vascular resistance of coronary arteries, epicardial arteries and arterioles that leads to rising of delivery of oxygen to a myocardium. The augmentation of a coronary blood flow caused by Plendil, promotes improvement of balance of blood supply/consumption of oxygen in a myocardium.
Plendil improves shipping of a load and reduces frequency of occurrence of attacks at patients with a stable stenocardia. There is a reduction of the symptomatic and latent ischemia of a myocardium at patients to a vasospastic stenocardia.
Indications
- An arterial hypertensia;
- A stenocardia.
Dosage regimen
Establish individually.
At an arterial hypertensia a preparation of 5 mg At insufficient hypotensive effect prescribe in an initial dose of 1 times/days a dose it is possible to enlarge or prescribe simultaneously other antihypertensive agent. The maintenance dose makes 5-10 mg/days At patients of advanced age there can be effective a dose of 2.5 mg.
At a stenocardia a preparation of 5 mg If necessary prescribe in an initial dose of 1 times/days a daily dose it is possible to enlarge to 10 mg.
The preparation should be accepted in the morning, without dependence from food intake. Tablets swallow entirely, not breaking and not chewing, washing down with water.
Side effect
From cardiovascular system: inflow, a tachycardia, puffiness in the field of an ankle joint, puffiness of gums (at patients with an ulitis and a periodontitis).
From party TSNS: a headache, giddiness, feeling of weariness.
At Plendil reception occurrence of following side-effects is possible, however an accurate relationship of cause and effect between reception of a preparation and occurrence of the given side-effects is not established.
From cardiovascular system: peripheric edemas.
From peripheric nervous system: paresthesias.
From the alimentary system: a nausea, vomiting, rising of activity of hepatic enzymes.
Dermatological reactions: a dermal eruption, an itch.
Allergic reactions: an urticaria, a Quincke's disease.
The other: an arthralgia.
Contraindications
- Pregnancy;
- A hypersensibility to a preparation.
Pregnancy and lactemia
Plendil is contraindicative to application at pregnancy.
Felodipine is allocated with thoracal milk. At Plendil application in a lactemia within recommended therapeutic doses degree of negative influence on the child is minimum.
Special indicatings
At appointment of a preparation as the patient with a stenocardia it is necessary to consider, that in rare instances Plendil can cause the expressed arterial hypotension which leads to a myocardium ischemia.
Plendil apply both to monotherapy, and in a combination with other antihypertensive preparations.
Plendil is necessary to prescribe with care to patients with a heart failure.
The preparation does not render influence on concentration of a glucose in blood and on a lipide profile.
At the patients transferred transplantation of a kidney and receiving cyclosporine, Plendil reduces a BP, improves a renal circulation, raises rate of a glomerular filtration and, besides, improves graft function.
Use in pediatrics
Data about Plendil application for children are not present.
Overdosage
Symptoms: the expressed arterial hypotension and a bradycardia is possible.
Treatment: spend symptomatic therapy. At the expressed arterial hypotension the patient should give horizontal position with the raised feet. At bradycardia development atropine introduction in a dose of 0 mg is shown i.v. If it is not enough of it, it is necessary to enlarge volume of plasma by infusion of a solution of a glucose, Sodium chloridum or dextran sodium. Sympathomimetic preparations with prevailing action on alfa1-adrenoretseptory prescribe at an inefficiency of the measures set forth above.
