Zofran (Ondansetron)Pharmacological action
Antiemetic preparation.
The action mechanism is bound to suppression of a vomitive reflex by blockade 5NT3-receptors at level of neurones both central, and peripheric nervous system. The prevention and treatment is based on this mechanism of action postoperative and caused cytostatic chemio - and radiotheraphy of vomiting and a nausea bound to rising of level of a serotonin which by activation of vagal eisodic fibers containing 5NT3-receptors causes a vomitive reflex.
Indications
- The prevention and elimination of a nausea and the vomiting, caused by carrying out cytostatic chemio - or radiotherapies;
- The prevention and elimination of a postoperative nausea and vomiting.
Dosage regimen
For the prevention and nausea and vomiting elimination at cytostatic chemotherapy. The daily dose for adults, as a rule, makes 8-32 mg. For preventive maintenance of the late or long vomiting arising through 24 h, it is necessary to continue Zofran reception in a dose of 8 mg 2 times/day within 5 days.
To children usually enter in the form of a solution for injections unitary i.v. directly ahead of the beginning of chemotherapy with subsequent Zofran reception inside in a dose of 4 mg through 12 h. After the termination of a course of chemotherapy it is necessary to continue reception of Zofran tablets in a dose of 4 mg 2 times/day within 5 days.
For the prevention and elimination of a nausea and vomiting in the postoperative period to adults recommend of 16 mg Zofran reception for 1 h before narcosis carrying out
To children for prevention and cupping of a postoperative nausea
At application for persons of advanced age of change of a regimen of dosage it is not required. At renal insufficiency it is not required to special correction of a dose, frequency of reception or a way of application.
At appointment of a preparation to patients with disturbances of function of a liver the preparation daily dose should not exceed 8 mg.
At Zofran appointment with a slow metabolism of a sparteine/debrisokvina of correction of a daily dose or frequency of reception it is not required to patients.
Way of application
Tablets for a resorption should not be squeezed out from a foil. Tablets should be got from packing directly ahead of the use. A tablet for a resorption place on a tongue end, after dissolution she needs to be swallowed.
Side effect
From the alimentary system: a hiccup, dryness in a mouth, constipations or a diarrhoeia, sometimes asymptomatic transient rising of activity of hepatic enzymes.
From cardiovascular system: pains in a thorax, in some cases with depression of segment ST, arrhythmias, a bradycardia, BP depression.
From party TSNS: a headache, giddiness, spontaneous impellent disorders and cramps, time disturbance of visual acuity.
Allergic reactions: an urticaria, a bronchospasm, a laryngospasm, a Quincke's disease, an anaphylaxis.
Contraindications
- Pregnancy;
- A lactemia;
- Children's age till 2 years (safety and efficiency of application was not studied);
- A hypersensibility to preparation components.
Pregnancy and lactemia
The preparation is contraindicative to application at pregnancy and in a lactemia (thoracal feeding).
Special indicatings
Since Ondansetron enlarges time of passage of contents on a colon, after preparation application the special control of a condition of patients with signs of impassability of an intestine is required.
Overdosage
There are limited data on an Ondansetron overdosage.
Symptoms: intensifying of collateral reactions in most cases became perceptible.
Treatment: spend the symptomatic therapy, the specific antidote is not present. To apply Ipecacuanha at Zofran overdosage does not follow, since its efficiency is improbable.
Medicinal interaction
There are no data that Ondansetron induces or inhibits a metabolism of other preparations often prescribed in a combination with it.
