Fucidin (Fusidic Acid)
Pharmacological action
The antibacterial preparation received from liquid Fusidium coccineum. It is highly active concerning Gram-positive bacteria (including Methicillinum-refractory strains Staphylococcus spp., Streptococcus spp., Corynebacterium minutissimum). The action mechanism is bound to suppression of synthesis of fiber at the expense of inhibition of the factor necessary for a translocation albuminous for a chain that results further in destruction of the originator.
Indications
For intake and i.v. introduction
Treatment of the infections caused by originators sensitive to a preparation, especially staphilococcuses:
- An osteomyelitis;
- A hematosepsis;
- An endocarditis;
- A pneumonia;
- Infections of a skin and soft tissues;
- A wound fever;
- Infectious-inflammatory complications at a mucoviscidosis.
For external application
- Anthraxes;
- Furuncles;
- Contaminated wounds;
- A hydradenitis;
- A folliculitis;
- A paronychia;
- A beard sycosis;
- An erythrasma;
- Ordinary eels.
Dosage regimen
Preparation in the form of tablets prescribe to adults and children of 500 mg 3 times/days are more senior 12 years in a dose
At infections of a skin and soft tissues - on 250 mg 2 times/days In serious cases the dose can be doubled.
Preparation in the form of suspension 3 times/sut, to children from calculation prescribe the adult on 15 ml of 1 ml (50 mg)/kg of mass of a body (parted on 3 receptions): at the age of 1-5 years - on 5 ml 3 times/sut; at the age of 5-12 years - on 10 ml 3 times/days
Preparation in a kind i.v. infusions 3 times/sut, to children - from calculation of mass of a body prescribe the adult on 500 mg of 20 mg/kg (parted on 3 introductions). The maximum daily dose for adults - 2 Duration of infusion - not less than 2 h.
The cream and ointment put on the amazed sites of a skin 2-3 times/sut within 7 days. At treatment of ordinary eels longer courses of therapy are required. Ointment can be applied with a bandage.
Rules of preparation of an infusional solution
For preparation of an infusional solution of a powder of 500 mg should be dissolved in 10 ml of an applied buffered solution. Further the received solution about 250-500 ml of sodium with Sodium chloridum for injections or 5 % a solution are necessary to dissolve Dextrosums for injections. The admixture of Fucidinum with a buffered solution cannot be entered not dissolved.
Side effect
From the alimentary system: at intake - a dyspepsia, a nausea, vomiting. At some patients after preparation reception (in the basic ambassador i.v. introductions in high doses) the reversible icterus became perceptible. If the icterus does not disappear, Fucidinum reception should be stopped.
The other: seldom - allergic reactions.
Contraindications
- A hypersensibility to preparation components.
Pregnancy and lactemia
Fucidinum gets through a placenta. Therefore it is necessary to avoid Fucidinum appointment in III trimester of pregnancy because of possible development of a nuclear icterus in newborns. The same fact should be considered at application of a preparation for newborns (including not full-term) children with an icterus and an acidosis.
Fucidinum is allocated with thoracal milk in very low concentration, therefore application of a preparation for feeding mothers is not contraindicative.
Special indicatings
It is necessary to consider, that Fucidinum is not toxic and can be applied at disturbances of function of kidneys and in cases when application of antibiotics is contraindicative.
It is noted a two-dimensional hypersensibility to other antibiotics.
At serious infections and also when long treatment is necessary, Fucidinum should be combined with others active concerning staphilococcuses antibiotics to lower risk of development of resistance.
Overdosage
At an overdosage spend symptomatic therapy. The dialysis is noneffective.
Medicinal interaction
At simultaneous application of Fucidinum with Penicillinums steady against action of penicillinases, cephalosporins, erythromycin, rifampicin, synergetic action is reached.
Pharmaceutical interaction
Solutions for the infusions, containing Fucidinum, are incompatible with Kanamycinum, gentamycin, Vancomycinum, Cefaloridinum, solutions of amino acids, blood-substituting preparations.
Conditions and periods of storage
The preparation should be stored in the dry place protected from light at a room temperature (from 15 to 25 C). The Period of validity - 3 years.
